Please arrive about 15 minutes earlier than your appointment time and check in with Krista, Audra or Christy in order to complete the registration process.

 

Please print and fill out the New Patient Form and bring it with you to your first appointment, along with your insurance card(s), photo ID, list of your current medications, and your physician referral (if you were given one). Physician referrals are not required for scheduling Physical Therapy in the state of West Virginia, however your insurance may.  Check with your insurance company to be sure, or if you have questions, we will be happy to help you understand your coverage.
